A blue rod is present in the markedly narrowed pulmonary artery (2 mm diameter). The right ventricular free wall (A, arrow) is hypertrophic (0.8 cm thickness). A large patent foramen ovale (not seen) was also present.

History: Fourteen year-old girl with cyanosis from age nine; physical examination showed a precordial thrill. Death was related to an attack of "acute dyspnea".
Comment: Pulmonary blood flow can be compromised by stenosis below, at or above the valve. It is usually accompanied by an intracardiac defect, such as patent foramen ovale, allowing blood to shunt from the right to the left side of the heart.
