The specimen shows a stomach with a large ulcerated carcinoma that has perforated (arrow) and formed a fistula with a segment of adjacent bowel (arrow on back view (B)).Â

Comment
The string adjacent to the perforation − particularly its basket-like appearance − is unusual, but was presumably put in place to keep the stomach-bowel fistula in place for the student to view. The case was reported by Dr J. A. MacDonald in the Canada Medical and Surgical Journal 1882 X: 405-408; Osler had performed the autopsy.
