"Problems of Living: Practising Psychiatry With Wisdom"

Abstract: Both philosophy and psychiatry are deeply concerned with “problems of living”. Kant asked “What can I know?”, “What must I do?”, and “What may I hope?”; questions that remain central in philosophy. In psychiatry, related questions arise about the nature of psychiatric diagnosis, the causes of mental illness, and the appropriate interventions for these conditions. It may be useful to contrast “Classical” and “Critical” approaches to questions about psychiatric diagnosis, etiology, and intervention; an “Integrative” approach that draws on the best aspects of these positions seems wisest. A number of “paradigm shifts” for psychiatry have recently been proposed, including by those working on clinical neuroscience, public mental health, evidence-based psychiatry, and positive psychiatry; instead an incremental and integrated approach to advancing the field may be optimal.
